How much does the Commonwealth Care Membership cost?

The price of your plan depends on the specific plan you choose. You can elect to pay your membership fee annually or monthly. By choosing to pay annually, you’ll receive one free month. The pricing for the annual membership reflects the one free month.

Adult Care Plan – $32/Month or $350/Year

Senior Care Plan – $28/Month or $310/Year

Child Care Plan – Starting at $25/Month or $280/Year

Family Care Plan* – Starting at $99/Month or $1,100/Year

Perio Maintenance Plan – $50/Month or $550/Year

Additional Child membership with a family plan – $21/Month per child or $220/Year per child
*The family plan is for an immediate family of four*

Is a Commonwealth Care Membership a dental insurance plan?

No, it is not a dental insurance plan and not intended to be a substitute for a dental insurance plan. The Commonwealth Care Membership is an affordable dental membership program that allows for a complete continuation of preventive to restorative services for those that do not have insurance or choose not to utilize their insurance to take advantage of special savings.

What benefits are included in the Commonwealth Care Membership?

The savings and benefits of both preventive and restorative care are significant. Depending on the plan you select, you can save more than $500 in your first year as an individual or more than $1,800 as a family of four. For a full list of the Commonwealth Care Membership benefits, click here.

How do I book an appointment with my Commonwealth Care Membership?

You can book your appointment online or call our office. When scheduling your appointment online, select “Commonwealth Care Membership” under the list of insurances. If calling our office, let us know you are a Commonwealth Care Member. Our office will confirm your appointment with you. To book an appointment online, click here.

How do I cancel my Membership?

The Commonwealth Care Membership can be canceled at any time after the first 90 days. To cancel your Commonwealth Care Membership, you can (1) come into one of our locations and talk to a member of our team or (2) call your preferred office location and speak to a member of our team. The credit card on file will be automatically charged monthly or annually, depending on the Membership selected, until canceled.

Can I get a refund if I don’t use my Commonwealth Care Plan?

The Commonwealth Care Membership is non-transferable and no refunds will be issued if the participant decides not to, or is unable to, utilize the Membership. Membership can be canceled any time after the first 90 days. If you decide to cancel during the first 90 days, you will receive a refund. If you utilized the plan in this time period and received treatment, you must pay for these services at our undiscounted usual, customary, and reasonable (UCR) fee schedule. If you selected to pay monthly, we will no longer charge the credit card on file once you cancel your Membership. If you chose the annual charge, you will not receive a refund after the first 90 days.

What are the Terms & Conditions?

The Commonwealth Care Membership is NOT A DENTAL INSURANCE PLAN and not intended to be a substitute for a dental insurance plan. The Membership can be canceled at any time but a refund will only be issued if canceled in the first 90 days. If services are rendered in the first 90 days and the Membership is canceled, you agree to pay in full for any services at our current usual, customary, and reasonable (UCR) fees. If Scaling and Root Planning (a.k.a. ‘deep cleanings’) are advised due to periodontal disease, the Perio Maintenance Plan will be recommended providing members up to 4 Perio Maintenance Cleanings and the member discount of 15% will apply to the Scaling and Root Planning procedure(s). You must be current on all Membership fees in order to receive any​ ​services included in the plan ​or ​​discounts on additional treatment. The credit card on file will be automatically charged monthly or annually, depending on the Membership selected, until canceled. Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts detailed in the plan. The Membership is non-transferable and no refunds will be issued if the participant decides not to or is unable to, utilize the Membership, or for any reason. The Commonwealth Care Membership is only valid for services rendered at our affiliated locations and cannot be used anywhere else. We use a third-party payment processor to collect and process all payments. It is your responsibility to provide accurate and up to date payment methods and information. If you do not provide a valid payment method, or if your payment method becomes inactive for any reason, we may immediately discontinue your enrollment in the Dental Membership ​Plan. We will attempt to reach out to you via text or email to gather up to date and accurate information prior to cancellation.  We reserve the right to change the fees associated with the Membership plan should increases in product or labor rise due to circumstances beyond our control. We will notify you at least 30 days prior to any such change or increase. Should default occur and a members account is assigned to collections, member agrees to pay and all fees including collection agency and other attorney fees (not to exceed 25%), court costs and any other fees advanced by this office to collect on any default.
